New Patients

Please complete all 6 (six) forms. Three forms (patient information, medical history and dental history) can be completed online and submitted electronically. There are three forms (consent to treat, financial policy and HIPPA) that require only a signature that can be printed off and brought to your appointment. By filling the out online forms and bringing in the signed forms, this will save time at your appointment. 

Recall/Cleaning Patients

Every six months we need an update on your child's medical history and this form can be completed online and submitted electronically. By completing this form prior to your appointment, this will allow us to bring your child right back and nothing to fill out when you arrive for their appointment.
